If you are residing in the United States and your Private Information was potentially accessed in the Dove Healthcare Management Services, LLC Data Incident in or around July 6, 2024, including all individuals who were sent notice of the Data Breach, you may be a Class Member and eligible for Settlement Benefits. Excluded from the Settlement Class are: (a) all persons who are directors or officers of Defendant; (b) governmental entities; and (c) the Judge assigned to the Action, that Judge’s immediate family, and Court staff.
If you are a Class Member and wish to receive Settlement Benefits then you must submit a claim form by July 7, 2026.
The proposed Settlement will provide the following benefits to Class Members:
Credit Monitoring and Insurance Services. All Settlement Class Members are eligible to enroll in two (2) years of one-bureau credit monitoring which includes up to $1 million of identity theft insurance coverage.
Documented Losses. Settlement Class Members are eligible to claim reimbursement of up to $3,000.00 per person for their documented losses resulting from the Data Incident including:
unreimbursed losses relation to fraud or identity theft;
professional fees including attorneys' fees, accountants' fees and fees for credit repair services; costs;
costs associated with freezing or unfreezing credit with any credit reports agency;
credit monitoring costs that were incurred between July 6, 2024, and July 7, 2026
Losses must not have been previously reimbursed or subject to reimbursement by insurance or a third party.
Lost Time. Class Members who spent time responding to the Data Incident may claim up to three hours, at $20.00 per hour, for a maximum of $60.00. You must have spent the time on tasks related to the Data Incident. Lost time is only recoverable if you submit a Valid Claim for documented losses.
You need to provide documented proof, like bank statements or receipts, to show how much you spent or lost. You can also provide notes or papers you made yourself to explain or support other proof, but those notes or papers alone are not enough to make a valid claim. Your proof or notes should show that your expenses were because of the Data Incident.
Pro Rata Alternative Cash Payment. Instead of a payment for either Documented Losses and Lost Time, you may claim a pro rata $50.00 cash payment. You do not have to provide any proof or explanation to claim this payment. The amount of the Alternate Cash Payment may decrease based upon the number of claims approved so as not to exceed the $150,000.00 cap on Cash Payments set forth in the Settlement Agreement.

The deadline to submit a Claim Form online or by email is July 7, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than July 7, 2026.
